URGENT!!! PA Long Term Care in need of LPN's!!!

Our Altoona based Skilled Nursing Facility is URGENTLY seeking Licensed Practical Nurses trained for ALL SHIFTS! Ideal candidates can assess, implement, and evaluate care needed by assigned patients under the direction of the licensed physician.

Schedule: Full-time, Part Time, PRN (FLEXIBLE SCHEDULING!)

PAY - $20-24/hr!!!!


Other : Graduate of an accredited school of nursing (Required)




Active Pennsylvania LPN License (Required)

BLS - Basic Life Support (Required)

Job Requirements

The responsibilities of the LPN are:

-Demonstrates responsible decision making in planning, providing and delegating care based on assessment. 

-Communicates effectively. Establishes and maintains two way communication with peers, staff, physicians and administration. 

-Adapts to change in a positive, professional manner. 

-Cooperates well with peers and supports group decisions. 

-Communicates appropriate information to charge nurse/supervisor/manager in a timely manner. 

-Documents accurately and timely according to guidelines. Writes legibly. 

-Maintains a clear, neat and safe environment for patients and staff. 

-Ensures supplies and equipment for patient care are readily available and organized in an efficient manner. 

-Correctly identifies and uses appropriate personal protective equipment. 

-Articulates the purpose of hourly rounding on patients. 

- Responsible for Rounding on Admissions and Transfers in the unit, Follow-up Rounding, Discharge Rounding, Service Recovery, Managing Up, Post-Rounding Communication with Staff and Documentation on Rounding Log. 

-Organizes and prioritizes elements of patient care for self and others. 

-Delegates appropriately tasks and duties to other members of the health care team. 

-Effectively plans interventions, evaluates response and reassesses plan of care. Documents response on the care plan. 

-Charting reflects patient problems identified in the plan of care and is timely and accurate. 

-Provides care to assigned patients including the activities of daily living, therapies and interventions. 

-Monitors/administers and charts all medications prescribed. 

-Monitors/reports in a timely manner any adverse drug reactions. 

-Performs all other duties and responsibilities as necessary.

Education: an associate's degree or equivalent, is required. 

License/Certificates: Current licensure as a Registered Nurse in good standing for the State of New Jersey required. AHA BLS required. ACLS required within six months of hire. Critical Care Course and Dysrhythmia required within one year of hire.
