The Oncology Nurse Navigator will function as a consistent and primary contact for the patients and their families as they enter the often complex cancer care system. The Cancer Center Nurse Navigator will assist in coordination of care across the continuum for a designated patient population and provides guidance as patients transition from one therapy to another.
The Oncology Nurse Navigator will ensure that the patient and family have appropriate information, understand and have the capacity to adhere to the medical plan of care. The Cancer Center Nurse Navigator will be the bridge between various consultants, services and points of care experienced by the patient/family.
The Oncology Nurse Navigator will interface with point of care staff in accordance with the plan for patient care, educate patients and families based on determination of learning needs and communicate plan details to patient/family, physicians, advanced practice providers, intra-professional team members and others involved in patient care.
The ideal Oncology Cancer Center Nurse Navigator will have BSN degree as requirement with MSN degree preferred. Oncology OCN certification preferred with minimum 3-5 years of RN experience in an oncology setting. Experience with policies/procedures, program development, data analysis, and physician relations.
